10 min | Incident Reports | Sagan | 7/19/22 - Patron emailed the library about some concerning things that happened to them while in the library.
- Indicated they had to flee Portland previously for similar concerns.
- Sandy follow-up with the patron; Sagan doesn't believe she has heard back from the patron.
- Not much for us to do at this point.

10 min | Near Miss Documentation | Sagan | Student was moving a book truck and accidentally knocked it over. Student was uninjured but made Sagan realize it might be worth having near miss documentation to help manage and prevent safety issues. - What are the group's thoughts?
- How do we want to communicate issues that aren't incident reports but a near miss.
- Kenna suggested updating incident report to include near misses.
- May be more relevant to RAS and Spaces? What are their processes.
- Have been a couple near misses in RAS that Derrick needed to document. Explained how Laura has been assessing/repairing/surplussing book carts over the past year. The difficulty was explaining that it wasn't a result of negligence.
- If we are seeing a trend, it might be important to do.
- How much work does it create?
- Rachel could ask around about this? Is there any kind of this type of reporting at this time?
- Sagan asked about liability implications of documentation.
- If we know something is functionally wrong and we keep using it, that's our responsibility.
- We need to have a clearly defined purpose for doing this.
- In other industries, it is to document to ensure things get wrapped up into trainings.
- Erin suggested that a Follow-up form could serve this purpose within LEAD.
- Will revisit this in the future perhaps.
- Discussed recognizing where there might be some overlap between departments.
- Karl asked if there is a place where faulty equipment should go? Such as faulty carts?
- Rachel said should be marked if faulty so it accidentally doesn't get put back into use.
- Lucinda suggested contacting library maintenance for triage and routing equipment to avenue for repair/surplus/etc.
- What other things that should probably be getting reported (non-book truck) that would work on the follow-up form.

- Want to create a culture of talking about safety openly.
- Should wait until we have another example of a clear miss before we come back to this and decide about documentation.
- Rachel summarized primary concerns related to near misses are equipment, security, and moving items.
- Lucinda suggested this could be grounds for creating training materials (wiki?) that all staff have access to use for training.

10 min | Safety Week Check-In | Kenna, Karl, Derrick | - Demo for fire extinguisher training scheduled for Oct. 11. Planning for out by the bike racks.
- Kenna is putting together a small proposal for purchasing stuff for safety week.
- Bicycle safety-ZAP will have a table for a day; bike registry program but may involve Public Safety.
- Planning to bring in therapy dog, Cedar, from Public Safety.
- HAM radio demonstration with Lise.
- Giveaways for staff.
- Scavenger hunt.
- Disaster films festival. The fun kind.

- De-escalation (Sagan)
| All | CPR/First Aid/AED - 20 people signed up.
- 3 sessions scheduled next week at Dixon.
- Lots of positive feedback. People have been waiting to renew their training.
De-escalation - Some of the trainings are being modified to work for LEAD.
- Sagan is using the curriculum as a starting point. Breaking it into regular sessions throughout the year.
- Sagan updated their licensing. Currently has 3 other seats for people to train with CPI.
- Also planning some sessions for in-library folks later this year.

| Need clarification on what the requirements are for shoe wear. What is the library's interpretation for OSHA requirements. - Previously told closed-toe shoes were required.
- Has not been enforced consistently across the library.
- Was discussed in LAMP in the context of dress code policy in general.
- A decision should probably be made at a library-wide level.
- Rachel suggested this may need to go back to LAMP. Has been a while since this has been discussed.
- OSHA requirements need to be interpreted. That should probably be done by LAMP.
- The question becomes do we require it for certain tasks as needed or for certain jobs at all times?
- Rachel will discuss with Kerri and Anne-Marie about taking it to LAMP.
